Referral Process

Placing a referral is easy.

We have several options you can pick from:

• Call us at (630) 420-0308.
• Fax over the patient demographic sheet, insurance and surgery or wound care information to (630) 420-0360.
• Send the referral over using CarePort/Allscripts (we are under “Premier Medical Products”).

Once we receive your referral, we’ll verify insurance and benefits, obtain any prior authorizations, contact the patient to coordinate the set up and track the referral from scheduling to equipment pick up at the end of the therapy.

For fax and All Scripts orders we’ll call or fax an order confirmation back so you know we have your order and are processing it.
